B.D.A. Evaluations:

Adults:

Mental Status Examinations are provided with referrals from the Bureau of Disability Adjudication. The time required for this evaluation is usually approximately 30-45 minutes, including the completion of a standard questionnaire. You will be asked about the history of disabling condition(s), to describe their current status, to describe your work history, and to answer questions about typical daily activities.

The mental status exam will involve brief testing of several types of "thinking" skills, including memory, reasoning, and general language skills.

The evaluation is provided at no cost to you. When the evaluation is completed, a report is sent to the Bureau of Disability Adjudication describing your present condition. The report is not a decision about your claim.

For your appointment, bring your eye glasses and/or hearing aids (if needed) and some form of identification. It will also be helpful if you bring a list of any medications you are currently taking.

Do not bring children under age eighteen. Adults, including adult children, are welcome to accompany you, particularly if their assistance may be helpful in providing history or helping complete required forms.

Children:

Mental Status Examinations for children typically include administration of individual standardized tests and are provided with referrals from the Bureau of Disability Adjudication. The time required for this evaluation will vary depending on the number of tests that are ordered but will typically be more than one hour. You will be asked to complete a questionnaire about your child, including the history and current symptoms of the disabling condition(s).

For the appointment, bring a list of medications the child is current taking. If there are school records that you think might help in understanding your child's condition, please bring them as well.

The evaluation is provided at no cost to you. When the evaluation is completed, a report is sent to the Bureau of Disability Adjudication describing your child's present condition. The report is not a decision about your claim.

TRICARE Evaluations:

Psychological evaluations are provided for military dependents and military retirees, typically after referral from a primary therapist or physician at the military hospital. The process begins with an initial assessment interview for review of the problem(s) which led to your referral. If psychological testing appears appropriate, a request to TRICARE for approval of the testing is submitted immediately following the initial session. A technician schedules the appointment and will provide details to you about the amount of time required. When testing is completed, a followup appointment to review and discuss the test results may then be scheduled in this office or the results can be forwarded to your primary therapist for the test interpretation.

For your first appointment, military referrals should bring photocopies (front and back) of TRICARE identification card and photocopies (front and back) of the beneficiary's military ID card. It is not necessary to bring copies of medical records. Do bring other information (school reports, etc.) which you believe would be helpful in understanding the problem.

As in most medical settings, the office area is shared with other providers during the week; child care is not available. If children (other than a patient) must come with you to the session, it will be helpful if a friend could accompany you to supervise during the session.

For military families, the co-pay fee is determined by your TRICARE program. In TRICARE PRIME, for example, there is currently no co-pay requirement for dependents of active duty personnel.

When a co-payment is required, TRICARE regulations do not allow forgiveness of the charge, and payment is expected at each session.
